Vaccination History:

We require proof of Rabies every 3 years, Distemper and Parvovirus (DHPP), and Bordetella. All puppies must have their third round of shots.

Note: It is your responsibility to keep us updated when you get your vaccinations renewed.

BORDETELLA (Kennel Cough)

The Coeur d‘Alene Pet Resort and doggie daycare requires the bordetella (Kennel Cough) vaccines. However, vaccinated canines are still susceptible to various strains of the virus. The vaccines for bordetella only cover a few strains of the virus.

I understand that although my dog is vaccinated against Bordetella, it is not a guarantee that my dog will not contract some form of the virus and will not hold The Coeur d‘Alene Pet Resort responsible for any veterinary expenses.

PLEASE NOTE:

***In case of any emergency situation or injury, you or your emergency contact number will be notified immediately. If immediate emergency medical attention is required, we will transport your dog to the nearest emergency veterinarian facility, along with contacting your veterinarian office. If the situation does not require immediate attention, we will contact you and your veterinarian and act in response via instructions. There is no cost to you for transporting your dog.

Please read the following and sign below:

  • I authorize The Coeur d’Alene Pet Resort to allow my dog(s) to engage in social play with other dogs. In doing so, I understand my dog(s) may become injured by interacting with other dogs. I also understand that The CDA Pet Resort and doggie daycare has evaluated the personalities of all dogs engaged in play, will supervise the playtimes, and will use reasonable care to ensure that no injuries result. However, in the event that an injury does occur to my dog(s) during group play, I agree to hold The Coeur d’Alene Pet Resort harmless from damages resulting from such injury. I agree to pay all veterinary charges required to properly treat my dog(s) for such injury.
  • I understand that I am solely responsible for, and hold The Coeur d’Alene Pet Resort and Doggie Daycare, LLC and their staff, NOT liable for any problem, harm or damage caused by my dog(s) or to my dog by another dog during his/her stay, provided reasonable precautions and care are provided and I hereby release The Coeur d’Alene Pet Resort and Doggie Daycare, LLC from all liability of any kind whatsoever.
  • I understand that all dogs at The Coeur d’Alene Pet Resort are vaccinated and healthy, but that there are viruses (e.g.,colds,bordetella) that cannot be fully vaccinated against and that in a social setting such as day care, my dog may be exposed to such.
  • I acknowledge and am aware that employees of The Coeur d’Alene Pet Resort and Doggie Daycare, LLC are NOT veterinarians and do NOT have backgrounds in animal medicine and are NOT expected to diagnose or detect illnesses in the pets that are being boarded at an The Coeur d’Alene Pet Resort and Doggie Daycare, LLC location. We will do our best to make sure your pet is safe and healthy.
  • If your dog requires medical attention while in the care of The Coeur d’Alene Pet Resort, you agree to be solely responsible for the payment of all medical bills for your dog and you release The Coeur d‘Alene Pet Resort , its officers and employees from any and all responsibility for, or claims, damages, debts, resulting from such medical care, including but not limited to, transportation to/from the veterinarian clinic and choice of veterinarian or animal hospital. There is no charge to transport to a local veterinarian.
  • There is a $1 per day charge when feeding your dog with “house” food (House food is Kirkland Chicken & Rice). Note: We recommend that you bring your own food to avoid your pet from having an upset stomach often associated with changing a dog’s food. Please bag your dogs food in individual servings.
  • We cannot guarantee that toys, blankets, or bedding will be kept in the same condition as brought in. To insure the return of all items, we mark your pet’s full name on all items unless previously marked or requested not to mark.
  • If a dog has fleas or other external parasites, the dog will be bathed with flea treatment at the owner’s expense.
  • I authorize to transport my dog(s) in a motor vehicle for purposes such as pick up and delivery, Veterinary visits and for any other such reason. I understand and agree to the risks associated therewith.
